BREED: Labrador Retriever   SEX: _____________ SIRE:  ______________________    DAM: ____________________   DATE OF BIRTH____________ LITTER NUMBER: ___________ COLOR: ___________________ MARKINGS: __________________ Sagebrush Labradors will hold registration papers until proof of spaying/neutering has been received from a licensed, reputable veterinarian. When proof has been received via a receipt and/or written statement from the vet, Sagebrush Labradors will forward the registration application papers and the sum of 50.00 to the purchaser's address. Spaying/neutering of this animal is required by the age of 6 months to receive the 50.00 incentive. Sagebrush Labradors must receive verification within one month from the date the procedure was performed otherwise registration of pup will be limited. The above contract will be signed and dated at the time of purchase and the registration papers held until proof of spaying or neutering has been done. We will then send you a 50.00 check along with your puppy's registration application.
